Important: Please read all sections carefully and sign where indicated. This will save you time during check-in.
Health Notice: If you answer "YES" to any health questions, we will send you a health form by email. This form must be reviewed with your personal physician and brought with you on the day of your dive, as it is required for insurance purposes.
